Ohio		Here is the Indiana ST-150 form that I always used in Indiana and I'm in Ohio.

Was the item a farm equipment? Because a Backhoe, Skid Steer Loader, Cars and Trucks, Tools, UTV's, Lawn Mowers, and such is not tax exempt in Indiana.

Also depends on Texas Sales Tax Exemption Laws.
