Here's my odd take. Even if he puts it in writing, what does that do if he retires/gets fired/goes broke in the next few years before problems show up? He gets his license revoked for too many botched procedures and she's paying someone else to fix his mistake. I'd be pretty inclined to get some legal advice on this one, at the very least a second opinion that things are and were done correctly.
